Periodontal Phenotype as a Predictor of Clinical Outcomes in Orthodontic and Implant Therapy: Current Evidence and Future Perspectives

Abstract
Periodontal phenotype has become a central concept in interdisciplinary treatment planning because soft- and hard-tissue dimensions may influence how periodontal and peri-implant tissues respond to orthodontic tooth movement, implant placement, prosthetic emergence, inflammation, and surgical augmentation. Yet the clinical literature often uses phenotype as though it were a fixed binary trait and, more importantly, as though association were equivalent to prediction. This critical narrative review evaluates whether periodontal phenotype can reliably predict clinical outcomes in orthodontic and implant therapy, identifies the components with the strongest evidence, and examines when phenotype modification is justified. Literature published from 1 January 1986 to 29 June 2026 was identified through live searches of major open scholarly sources, supplemented by citation chaining and verification of bibliographic metadata and digital object identifiers. Evidence was synthesised according to phenotype definition, measurement validity, treatment context, outcome domain, confounding, follow-up, and the distinction between prognostic association and intervention effect. In orthodontics, thin gingiva, limited keratinised tissue, pre-existing recession, and a constrained facial alveolar envelope consistently emerge as vulnerability markers, but the independent predictive value of any single phenotype measure remains uncertain because tooth-movement geometry, baseline anatomy, inflammation, age, and measurement method are incompletely controlled. In implant therapy, reduced mucosal thickness and keratinised mucosa are associated with selected early marginal bone, inflammatory, comfort, and soft-tissue outcomes, although implant position, abutment geometry, timing, and maintenance can modify or outweigh these associations. Soft-tissue augmentation has a clearer evidence base around implants than before orthodontics, but phenotype modification should not be treated as universally preventive. Overall, periodontal phenotype is best interpreted as a modifiable, site-specific contextual risk indicator rather than a deterministic predictor. Future progress requires continuous measurements, standardised landmarks, prospective multivariable prediction studies, external validation, and trials designed to test whether changing phenotype changes clinically important outcomes.
